We need all of the items in order to start setting up your account for credit card processing. Once your application is complete, email it to applications@braintreepayments.com or fax it to 1-847-890-6644. We don't need a copy of the fine print - only pages you have filled out or signed. If you haven't heard back from us about your application within 1 business day, please contact your account manager to make sure it came through.

The pdf is interactive, so after you download the file you can fill out most portions on your computer before you print and sign the document (Please note: If you don't have Adobe Acrobat, or another method to edit pdfs, you won't be able to save filled out information).

Please sign and return our Services Addendum, which clarifies that you may cancel at any time without incurring any termination fees.


This outlines your vault and recurring billing fees - you'll only be billed if you use these services.

5. Voided Check / Letter

If you don’t have checks, don’t worry. Send us a bank letter instead (see our sample below). Be sure the letter is signed by your bank on bank letterhead, and confirms your routing and account numbers.

NOTE: In either case, this is the account where we'll deposit your funds and debit our fees.
