The pdf is interactive, so after you download the file you can fill out most portions on your computer before you print and sign the document (Please note: If you don't have Adobe Acrobat, or another method to edit pdfs, you won't be able to save filled out information).

Merchant Application Addendum - clarifying that you may cancel whenever you want and we'll never charge any termination fees.
Discover is now included with Visa and MasterCard on the same monthly statement which will make life easier for you. Please sign the Confirmation Page from the Discover Terms of Service below and return with the rest of your application materials.
If you don’t have checks, don’t worry. Send us a bank letter instead (see our sample below). Be sure the letter is signed by your bank on bank letterhead, and confirms your routing and account numbers.
NOTE: In either case, this is the account where we'll deposit your funds and debit our fees.
