We need all of the items in order to start setting up your account for credit card processing. Once your application is complete, email it to applications@braintreepayments.com or fax it to 1-847-890-6644. We don't need a copy of the fine print - only pages you have filled out or signed.

1. Merchant Application

The pdf is interactive, so after you download the file you can fill out most portions on your computer before you print and sign the document (Please note: If you don't have Adobe Acrobat, or another method to edit pdfs, you won't be able to save filled out information).

If you want to start accepting American Express, please sign on the second page of the application, and we’ll set it up.

Discover is now included with Visa and MasterCard on the same monthly statement which will make life easier for you. Please sign the Confirmation Page from the Discover Terms of Service below and return with the rest of your application materials.

2. Voided Check / Letter

Voided Check preprinted with legal or dba name and address

If you don’t have checks, don’t worry. Send us a bank letter instead (see our sample below). Be sure the letter is signed by your bank on bank letterhead, and confirms your routing and account numbers.

NOTE: In either case, this is the account where we'll deposit your funds and debit our fees.

4. Previous Statements

Provide three months of previous Processing Statements

Only if you’ve already been processing credit cards.
